If your child has been prescribed medication we are able to administer this in school. Medication must have been prescribed for the current illness.
Please note that we are also able to administer "over the counter" non-prescribed medicines.
If preferred, parents / carers are welcome to come to school to administer medication.
Please print off and complete the form below and bring it along to the school office with the medication (please do not give it to your child to hand in). Alternatively, you can leave the medication and form with a member of GOOSE staff.
